I've done 10+ FMT's and tried a few different approaches to see if I can increase its effect. One transplant I did lasted about a year with good results and would have lasted longer if it weren't for me doing some more experiments since it didn't fully cure me. I'm not sure if it was thanks to the donor or due to my procedure, but I'll share what I did so more people can try.
Day before the transplant: Bowel prep. Normally I use osmotic laxatives because that's what's always used in studies, but I tend to remain a bit bloated even after the prep, so I tried using stimulant laxatives (bisacodyl) this time because my stomach becomes flat when I use that. I don't know which laxative is the best but in my experience bowel prep does increase the effect of FMT. This also makes sense theoretically since about 55% of our stool is bacteria, so emptying the gut before the transplant means less competition, metabolites and gasses that can interfere with the new bacteria.
This day I did not have anything to eat, only water and a tiny bit of salt for some electrolytes.
Day of transplant: I did the FMT in the morning (upper route) while fasted and did not eat or drink anything afterwards (only a tiny bit of water at the end of the day). The purpose of this is to allow the new bacteria to colonize undisturbed and for their metabolites to have some time to modulate the old biome without interference. I've noticed that there seems to be activity in the gut during sleep as well so that's why I wanted the new microbiome to get one full cycle through the system without interference. Maybe that allows the new one to dominate the gut better.
Day after: Start small with food that you know you can tolerate well and gradually build up afterwards.
This treatment had some of the most profound effects I've experienced from FMT with improved gut symptoms, energy, libido, tolerance for foods and some dramatic changes in personality. This donor had a very dominant personality and I took on this trait as well (I literally went from being non confrontational to very confrontational and dominant). I've actually had the best results from donors with strong personalities (maybe strong personalities makes the donor more potent?). So I don't know whether it was just the donor or the procedure that made this treatment stand out so much.
And since n=1 it is impossible to draw any conclusions from this, but if anyone is going to do an FMT give this a try and report back with your results.
